This notice describes the ALVERA contact form and how details submitted to request an online consultation are handled.

Your enquiry

The form requests your name, email, phone number and departure city so we can respond to your enquiry and coordinate first contact. Submission does not confirm an appointment: the clinic must confirm availability.

When you submit, the four contact fields and page language are transmitted to FormSubmit, which forwards the enquiry to ALVERA’s dterziu2@gmail.com inbox, hosted on Gmail. This is not a subscription to marketing messages.

The page code does not save these form values in cookies or browser storage. Your browser may apply its own autofill settings. FormSubmit states that it retains submissions for 30 days. The retention period in ALVERA’s inbox has not yet been specified in this notice.

Do not send health information

The form does not request or support uploads of X-rays, photographs, diagnoses, medical history or identity documents. Do not enter health information in the text fields. The clinic will separately define how any clinical documents should be shared.

Browser and external services

Choosing an edition stores only your market preference under alvera.market in your browser’s local storage. This contains no form or health information. There is no IP-based geolocation or inference of nationality. You can change the edition at any time.

Images, 3D models, libraries and fonts are local or system resources. The page code does not include advertising pixels, behavioural analytics, remote fonts or embedded maps and social feeds. External links open only when selected. Hosting, FormSubmit and Gmail may process technical information under their own terms.

Privacy contact and outstanding details

For questions about your details or requests for access, correction or deletion, write to dterziu2@gmail.com. The controller’s full legal identity, legal basis, any international transfer details and ALVERA inbox retention still need to be specified; this notice is not a completed legal review.
